• Unless already provided, please bring copies of birth certificate and USA Hockey membership information.

• $295 team/program fee* in cash or check (made payable to: SCVHA) will be required.

5. Monday 5/12 – 6:00-7:15pm at Fremont

6. Thursday 5/15 – 6:00-7:15pm at Fremont

7. Monday 5/19 - 6:00-7:15pm at Fremont

*Program Cost:  $295 (due by 5/1) which includes 7 practices, tournament fees and coach’s stipend.
